1. We will receive your version of the form, convert it into a fillable PDF, and upload it as an electronic document on the Child Profile Page.

2. Whenever you need to make a claim, you can go to that child’s Profile Page and start a new document.

3. A partially pre-filled CMS-1500 form will open, and you can manually complete the remaining information.

4. After filling in the details, save the form.

5. Each child’s profile will store multiple dated versions of the form, allowing you to keep track of all submissions.

6. You will also be able to print or email the completed forms.
